The following Employment Opportunities are currently available within Hospice of Huntington:

Full-Time Medical Social Worker*

Shift:  Tuesday through Saturday

DUTIES:

  • Social work evaluation of patient/family psych/social and environmental needs
  • Implements the social work treatment plan and documents consistent with insurance guidelines in their homes or facilities
  • Assists the primary nurse with establishing and maintaining a treatment plan for each patient/family.
  • Provides support to selected patients post discharge.
  • Represents the philosophy and services of Hospice of Huntington, Inc.

POSITION QUALIFICATIONS:

  • Bachelors Degree in Social Work from an accredited school of social work required, a Masters Degree in Social work preferred.
  • West Virginia and Ohio Social Work license are required. 
  • Valid driver's license along with an insured, reliable vehicle required.
  • Minimum of one year experience in healthcare, with a preference in Hospice/Home Health Care.
  • Evidence of clinical experience and skills pertinent to assessing and counseling individuals.

Full-time or Part-time Palliative Care Nurse Practitioner

The Nurse Practitioner assesses treats, solves problems, counsel, and educates hospice and palliative care patients, utilizing advanced assessment and evaluation techniques for home based palliative care patients.

DUTIES:

  • Performs consultative visits for certification and recertification assessment
  • Performs consultative visits for pain and symptom management
  • Offers clinical changes to plan of care with collaboration from attending physician and/or medical director.  Offers clinical nursing changes to plan of care and communicates to hospice care team
  • Prepares and provides staff training and medical and community training on relevant clinical aspects and end-of-life philosophy and care
  • Confers with medical directors to review diagnosis, assessment criteria, and issues of pain and symptom management for population served
  • Initiates any immediate orders with consult of the attending medical director
  • Assesses health status and health potential:  plans, implements and evaluates health care needs of the patient/family
  • Plans and evaluates therapeutic regimens for patients, to minimize pain and suffering, consulting with physicians and other members of the health care team, and community agencies


POSITION QUALIFICATIONS:

  • Completion of Accredited Nurse Practitioner Program
  • Masters Degree in Nursing
  • Current license to practice as a registered nurse in Ohio and West Virginia
  • Current certification as family or adult nurse practitioner
  • Two years experience of practice in the Nurse Practitioner Role, geriatric experience preferred

Position Title:  Registered Nurse, or LPN

Full-Time 

*Must have flexibility as evening and weekend hours may be required
* May be required to work in the Home Program as needed

  • Perform assessment of assigned patients and derive nursing diagnoses based on assessment data.
  • Plan nursing interventions in collaboration with the interdisciplinary team to resolve current problems or prevent potential problems.
  • Provide teaching to patients/families to assist in the patients’ comfort as needed.
  • Provide emotional support to patients/families in a manner that fosters independence and affirms existing strengths.
  • Assess the need for home health aide services.
  • Assess and provide pain management strategies to assist with the patient’s comfort.
  • Promote working in a team environment.
  • Comply with policies and procedures of Hospice of Huntington, Inc.
  • Protects the confidentiality of private health care information of patients.
  • Promote the philosophy and services of Hospice of Huntington, Inc.

  • Graduate of an accredited professional school of nursing; BSN preferred.
  • Licensed registered nurse in West Virginia and Ohio.
  • Minimum of 2 years nursing experience; home health or hospice nursing preferred.

Full-time Hospice Aide for Home Program*

DUTIES:

  • Performs and/or assists the patient with personal care activities as prescribed on the Hospice Aide plan of care.
  • Performs and/or assists the patient/family with light housekeeping and meal preparation.
  • Reports patient status changes to the interdisciplinary team as needed.
  • Maintains current and accurate documentation.
  • Complies with policies and procedures of Hospice of Huntington, Inc.
  • Protects the confidentiality of private health care information of patients.
  • Represents the services and philosophy of Hospice of Huntington, Inc.

POSITION QUALIFICATIONS:

  • High school graduate is preferred.
  • Proof of nursing assistant certification.
  • Minimum of 1-year experience, preferred
  • Current driver’s license, automobile insurance, and reliable transportation.
  • Philosophy compatible with the hospice concept.
  • *May be required to work at Emogene Dolin Jones Hospice House, as needed.

Position Title: RN Team Manager Southern Ohio Nursing Home Team

Employment Type: Full-Time

  • Assesses eligibility of potential hospice patients in collaboration with the interdisciplinary team.
  • Initiates the physical assessment and plan of care for each patient admission

  • Promote the philosophy and services of Hospice of Huntington, Inc.
  • Other job duties as assigned
